ASRM Issues New Guidance on Fertility Care During COVID-19 Pandemic

Washington DC – The American Society for Reproductive Medicine (ASRM), the global leader in reproductive medicine, today issues new guidance for its members as they manage patients in the midst of the COVID-19 pandemic.  Developed by an expert Task Force, of physicians, embryologists, and mental health professionals, the new document states current recomendations.

Specifically, the recommendations include:

  1. Suspension of initiation of new treatment cycles, including ovulation induction, intrauterine inseminations (IUIs), in vitro fertilization (IVF) including retrievals and frozen embryo transfers, as well as non-urgent gamete cryopreservation.
  2. Strongly consider cancellation of all embryo transfers, whether fresh or frozen.
  3. Continue to care for patients who are currently ‘in-cycle’ or who require urgent stimulation and cryopreservation.
  4. Suspend elective surgeries and non-urgent diagnostic procedures.
  5. Minimize in-person interactions and increase utilization of telehealth.

The above recommendations will be revisited periodically as the pandemic evolves, but no later than March 30, 2020, with the aim of resuming usual patient care as soon and as safely as possible.  ASRM has been closely monitoring developments around COVID-19 since its emergence. Data on its impact on pregnancy and reproduction remains limited.

Treatment Policy in light of Coronavirus COVID-19

RMSCVA is closely monitoring developments on both global and domestic fronts regarding the coronavirus (COVID-19). The health and safety of our patients and staff remains our top priority. We are following the guidance of the CDC, FDA, ASRM and local public health authorities as we develop policies to maximize the safety of our patients and staff while still providing excellent care. We have summarized the latest  guidance into our policy but it is a rapidly changing landscape and recommendations may change daily.

 

Patients who meet the criteria below should postpone their fertility treatment at our center, including egg retrieval and embryo transfer cycles:

 

  • Any patient who has traveled to, from, or through mainland China, Italy, Iran or South Korea within the past 30 days (additional countries or regions may be added as the situation changes). We will define COVID outbreak areas as identified by the CDC as Level 3 Travel Notice countries/regions. Many European nations have recently been added to the list.   • Any patient who has come into contact with a person who has traveled to or through one of the countries listed above within the past 30 days (CDC characterizes contact with an individual as coming within six feet of a person).

 

  • Any patient who has been in contact with an individual who tests positive for COVID-19 within the past 30 days.

 

  • Any patient who within the past 30 days has had contact with anyone suspected of having the COVID-19 virus.

 

  • Any patient who for any other reason who has been recommended to be in quarantine for the COVID-19 virus.

 

Patients with fever, cough and/or upper respiratory symptoms but not diagnosed with COVID-19 should be evaluated by your primary care provider or the emergency department or follow guidelines for evaluation for Covid-19 in your community. DO NOT COME TO OUR OFFICE IF YOU HAVE ANY OF THOSE SYMPTOMS AND HAVE NOT BEEN EVALUATED FOR COVID-19, OR IF YOU MEET ONE OF THE CRITERIA LISTED ABOVE. WE DO NOT WANT TO EXPOSE OTHER PATIENTS OR OUR STAFF.  If you have any questions, please call to talk with us to determine if we can continue to treat you or to let us know that you recognize that you will need to discontinue your treatment this cycle.

 

Patients that have traveled out of the country within the past 30 days, and are not on the CDC list for Level Three Countries (which excludes you from care for 14 days) are asked to please wear a mask while in our office to protect themselves and other patients.

 

If a patient has already started a cycle and meets the above criteria for cycle postponement, we will require cycle cancellation (including cancelling an egg retrieval).

 

Donor Egg and Gestational Carrier Cycles

 

Additionally, per FDA recommendations, egg donors and gestational carriers will receive additional screening regarding COVID-19 risks within the past 28 days. A donor or gestational carrier cycle may be cancelled for the following activities within the past 28 days:

  • Travel to areas with COVID-19 outbreaks, as defined by the CDC
  • Living with individuals diagnosed with or suspected of having COVID-19 infection
  • Been diagnosed or suspected of having COVID-19 infection

 

Other Guidelines

 

If possible, please go alone to any appointments. Do not bring children, family members or friends unless you need assistance. Especially, do not bring anyone who has a cough, a fever or shortness of breath.

 

Please be aware that the COVID-19 crisis is very dynamic and policies are procedures are subject to change as the climate of this pandemic changes. We will do our best to update the policy in a timely fashion.
